Preparation of an ICU Patient for Transfer to LTV Unit Developed by the LTV Patient Transfer Working Group through the Toronto Central Local Health Integration Network, these guidelines were created to assist in facilitating a smooth transition for invasively-ventilated patients from the critical care setting to suitable long-term ventilation beds in non-acute settings.

Our Home Ventilation Training Pre-Assessment and Chronic Assisted Ventilatory Care Pre-assessment forms are now available on-line and can be downloaded and printed. These forms are not meant to replace the referral to either of these programs. Once a referral has been received, the applicant will participate in a Pre-Assessment by our interdisciplinary team to ensure that the eligibility criteria are met, prior to approval for admission.
