A new resource for healthcare professionals: Preparation of an ICU Patient for Transfer to LTV Unit Developed by the LTV Patient Transfer Working Group through the Toronto Central Local Health Integration Network, these guidelines were created to assist in facilitating a smooth transition for invasively-ventilated patients from the critical care setting to suitable long-term ventilation beds in non-acute settings.
